and related services. Same day appointments are usually
available.
Transportation to and from our clinic is available
for patients who have been involved in a crash when
their auto is unavailable due to the crash.
Language interpreting services are available.
Preparation for Your First Appointment
Bring the following information to your first visit:
1. Auto Insurance information: please bring in your
Proof of Insurance card or a copy of your card.
2. General Health Insurance information: please bring
in your subscriber’s card or a copy of your card.
3. Listing of Medications: please bring in a complete
listing of all medications, including the dosage you
currently are taking.
4. Listing of Previous Health Care Facilities/Doctors:
please bring in a listing of all health care facilities
and/or doctors that you have seen or are seeing for
this condition.
5. Accident Report: please bring in a copy of your accident
(police) report , if you have been in an automobile
crash.
Plan about one to one and a half (1–1 1⁄2)
hours for the first appointment.
What to Expect at Your First Appointment
During your first chiropractic consultation, expect
the following:
1. You will be asked to complete a case history form.
If you need assistance, staff at our front desk is available
to help you.
2. An initial physical examination will be performed
which usually includes chiropractic, postural, neurological
and orthopedic exams.
3. If indicated, x-rays may be taken. Wear comfortable,
metal free clothing; be prepared to change into a patient
gown if necessary. The valuable information that is
obtained from your examination will help to determine
whether or not chiropractic care will be beneficial
in your case and your expected outcome.
What to Expect at Your Second Appointment
On your second visit, plan about one (1) hour to:
1. Review with you, your spouse, and/or other family
member (s) the results of your exam.
2. Discuss our proposed treatment plan, which is based
on your diagnostic impression.
3. Answer any of your questions or concerns.
4. Have someone from our insurance department available
to answer questions regarding payment issues.
5. In most cases, you will receive your first chiropractic
treatment.
